At Comprehensive Collaborative Care, we offer specialized outpatient mental health services with a strong emphasis on collaborative care. Our goal is to achieve the best possible patient outcomes by integrating the expertise of our multidisciplinary providers. We work to ensure accurate diagnoses and swift stabilization through a combination of psychotherapy and pharmacological management, emphasizing a collaborative approach.

Unlike other practices, our stabilization phase involves consultation with two providers at once: a clinical psychiatric pharmacist, who focuses on optimizing medication regimens and a board-certified psychiatric nurse practitioner, who handles medication management, as well as performing an in-depth psychiatric evaluation, to help in establishing a diagnosis. Additionally, our Clinical Psychologist specializes in diagnostics, psychotherapy, psychological and neuropsychological assessments and is available for consultation.  We have other skilled staff members available for ongoing psychotherapy. By leveraging each professional's expertise to its fullest, we deliver comprehensive and effective care.

Our Clinical Pharmacist and Psychiatric Nurse Practitioners see patients together, integrated with our Clinical Psychologist to provide comprehensive, personalized care. Stephen Gibson, a Clinical Psychiatric Pharmacist and the founder of Comprehensive Collaborative Care, collaborated with many psychiatric nurse practitioners while working at a psychiatric hospital for adults in New Hampshire. Together, Stephen and the nurse practitioners managed complex cases involving mental health disorders. The effectiveness of their collaboration was evident in challenging cases, where a team approach proved far superior to a single practitioner approach.​

As a team, they addressed patients' concerns, identified side effects causing discomfort, explored suitable medication options, and empowered patients to participate in their own care. Patients were involved in decisions about their medication regimens and overall care.  When patients felt heard and understood, it led to faster and more timely discharges. However, a significant missing component in this care was the lack of psychotherapy support.

Our innovative 2-on-1 approach featuring a Doctor of Pharmacy and a Psychiatric Nurse Practitioner working side by side, has made a profound impact on the treatment of mental health. Building on our collaborative success, we have taken a significant step forward in transforming outpatient psychiatric care.  By integrating a Doctor of Clinical Psychology to work closely with the medication team, we are able to enhance diagnostic accuracy, provide comprehensive psychotherapy, as well as psychological and neuropsychological assessments.

This integration of specialties has led to a truly client-centered care approach and success in client outcomes. Our unique outpatient model is pioneering in New Hampshire. We focus on fully understanding our clients from all perspectives using our different areas of expertise, involving the client in their treatment, and addressing any concerns they may have.

At our practice, patients benefit from consulting with multiple providers at once, allowing for open discussions about medications, side effects, and therapy adjustments. Client involvement and input is central to our approach. By working together with our dedicated team, you can experience the highest standard of personalized care.

Our Patient Care Approach

In our comprehensive collaborative care approach, we focus on accurate diagnoses and effective treatment. The process starts with patient enrollment in our portal, where intake paperwork and screening questionnaires provide essential background information. This data assists our detailed intake appointment, during which we evaluate and discuss the client’s medical and mental health history. Based on information obtained during this session, we offer personalized recommendations, including options for psychotherapy with either our Clinical Psychologist or other psychotherapists on the team, a combination of psychotherapy and pharmacological management, or direct medication management tailored to the patient’s diagnosis.  Psychological and neuropsychological evaluations are also available when indicated.

We emphasize a shared decision-making model, ensuring a client-centered approach. If patients opt for psychotherapy alone, we support their choice and work together to achieve the best outcome.

If psychotherapy alone does not achieve optimal results, additional interventions like medication management may be recommended to effectively reduce symptom severity.

Our unique two-provider approach to medication management involves both a psychiatric clinical pharmacist and a psychiatric nurse practitioner working with one client simultaneously during the acute phases of care to stabilize medication regimens. After stabilization, patients continue their medication therapy management with either the clinical psychiatric pharmacist or one of the psychiatric nurse practitioners, ensuring ongoing comprehensive care. Psychotherapy is provided either by our clinical psychologists or licensed clinical mental health counselors.  Clinical Psychologists provide all psychological, and neuropsychological assessments.

We are committed to evidence-based medicine, using systematic assessments to evaluate treatment responses and adjust care as needed. We carefully balance psychotherapy and medication management, customizing our approach to each client’s diagnosis. Psychotherapy is our preferred initial treatment, but we seamlessly transition to medication management or other modalities, if necessary, to enhance patient outcomes.
